Cataract is a clouding that develops in the crystalline lens of the eye or in its envelope, varying in degree from slight to complete opacity and obstructing the passage of light. When a cataract is sufficiently developed to be removed by surgery, the most effective and common treatment is to make an incision (capsulotomy) into the capsule of the cloudy lens in order to surgically remove the lens. Following surgical removal of the natural lens, an artificial intraocular lens implant is inserted (eye surgeons say that the lens is "implanted"). There are no less than 30 types of modern IOL at the disposal of ophthalmologists. However most of them have specified parameters of magnification. in other words the patient who is going to undergo IOL implantation has to choose between good visual acuity into the distance or good near vision.
Elasticity of the eye's crystalline lens provides changing the power of the lens that is why we can see distant and nearby objects. And only multifocal lens can completely substitute the lost abilities of crystalline lens.

However the high quality of lens requires good vocational training of operating surgeons. Five components of success without which even the most expensive lens will be useless:




1. Surgical equipment of the latest generation.

2. Special training of surgeons.

3. Exact calculation.

4. Modern viscoelastics.


5. Multifocal IOL with gradient optics.

Besides the calculation of lens parameters needs exact diagnostics. And also safety: we use only single-use instruments and materials.

     Multifocal IOL with gradient optic “Gradiol” is a product of several fundamental branches of science
What are the advantages of this lens in comparison with other ones? We mark out five distinctions:
1. the device is simple in construction, easy to be inserted and safe during post-op period
2. the loss of light energy in the transition zones and the steps of diffractive lenses is reduced
3. the dependence of the functional result on the size of the pupil is reduced
4. the risk of developing of light effects during post-op period is reduced
5. the cost of IOL is reduced due to single-stage technology in comparison with worldwide practice of two-stage production (IOL is cut out form polymer which was produced before).
Our research took a lot of time and resources. It lasted 5 years. Now we have industrial model of the lens which has proved to be effective, safe and competitive.Thus basing on scientific traditions of our institute originating in the work of S.N. Fyodorov we created new high-tech method of cataract extraction with implantation of multifocal IOL “Gradiol”.




     Let’s depict and eye with gradient IOL in diagram form. It is not so easy because the lens forms 2 focuses on the cornea. But human optic system includes not only eyes. Two images are formed on the top layer of retina – of distance and near objects. And the brain chooses the image according to its priorities and functional needs. So we see by our brain integrating the visual information.
We had to create a special computer program in order to simulate optic processes inside the eye and even on the top layer of retina. Thus even before the implantation of the lens we knew how would the patient see after the surgery.



     
At our institute we have all the necessary equipment and our experienced surgeons make a puncture which is... 1.8-2.2 mm in breadth! Can you find smaller one? Through this puncture the surgeon removes the dim lens and inserts a new multifocal IOL. No section, no pain, no scars and long period of regeneration. The surgery lasts about 10-15 minutes. It is followed my a comfortable rehabilitation and a day later the patient can return to his life full of activity and energy!

P.S. Not long ago a pilot from Arkhangelsk complaining for cataract visited our institute. Medical board did not allow him to fly. Imagine what he felt! He was successfully operated in our institute and now he continues his work! We are also very proud of the fact that we helped our several colleagues – neurosurgeons.
