     In June 2004 the Laser Surgery Center of the Eye Microsurgery Complex celebrates the 30th Anniversary from the day of establishment.
     The Center is the biggest one not only in Russia but has no analogues in the world in volume and range of performing laser surgeries.
     A.D.Semyonov – the founder and first chief of the All-Russian Center of Laser Surgery and Fluorescent angiography, Doctor of Medical Sciences (M. D., Ph. D.), Professor, Academician of the Laser Sciences Academy, Honoured Doctor of the Russian Federation headed it during 25 years.
     In our country and abroad A.D.Semyonov is known as one of the pioneers of laser eye microsurgery who created together with his staff a range of effective methods for diagnostics and treatment of eye diseases with use of the laser radiation energy. He is a founder of the new trend in ophthalmology – using of the low energy laser impact for the stimulation of retinal functions, as well of the cornea and the other eye tissues, a founder of the refractive laser surgery in the USSR and in Russia, creator of several generations of the Russian surgical lasers “Profile” and “Microscan-2000”.
     A.D.Semyonov became one of the authors of industrial technology for diagnostics and surgical treatment of the patients with eye diseases first developed in the world. He is the author of 55 inventions and patents, 48 rationalization proposals, over 300 scientific works, including 2 monographs. Under his supervision were fulfilled 12 dissertations for the degree of candidate of medical science and 3 doctoral dissertations.
     Since 2001 till the present moment G.F. Kachalina candidate of medical sciences (M. D., Ph. D.) is the head of the Laser Surgery Center.
     She is the creator of the domestic excimer laser technology, which was presented in her dissertation “Surgical Technology of transepithelial PRK in myopia with use of excimer laser unit “Profile-500”. She published 81 scientific articles, received 11 patents for inventions. She is actively engaged in practical activity having consulted and operated over 5 thousand patients. Several times she was sent by the Institute to work abroad.
     Annually from 8 to 11 thousand operations were performed at the Center, and for the last years over 30 000 patients were treated there.
     The whole spectral range of laser irradiation applied for ophthalmic practice is used in these procedures.
     At the Center work 18 highly qualified laser surgeons, among them 1 Academician, 4 Doctors of Medical Sciences, 8 Candidates of Medical Sciences (M. D., Ph. D.).
     The Center includes refractive surgery department, research laboratory of laser and optic investigations where talented scientists and engineers work; the department of the fundus oculi and anterior eye sections laser treatment methods, department of fluorescent angiography.
     The Laser Surgery Center of the SI IRTC Eye Microsurgery is a pioneer in excimer laser microsurgery of the eye.
     Experimental investigations in this trend began as far back as in 1984. The first domestic ophthalmic excimer laser unit which forming system has no analogues in the world was developed in 1986.
     In 1988 simultaneously with German and American ophthalmologists the first excimer laser refractive operations at the clinic were performed at the SI IRTC Eye Microsurgery.
     For the period from 1988 to 1997 several generations of the domestic ophthalmic laser unit “Profile” were created, the last of them “Profile-500”, and over 60 thousands photorefractive operations (PRK) were performed in cases of refractive anomalies.
     In 1992 at the Laser Surgery Center were performed the first “LASIK” operations. Since that time the technology of this operation is being permanently perfected and the quantity of the operations grows every year.
     In recent years at the Laser Surgery Center was developed one more technology of excimer laser microsurgery in refractive anomalies, the main element of which is the first domestic scanning excimer laser unit “Microscan” with original technological decisions included into it and working on the “flying spot” principle.
     The developed technology allows to correct according to keratotopography data mild, moderate and high myopia, hyperopia, hyperopic and mixed astigmatism as well as ametropia after operations performed earlier, injuries and other corneal diseases.
     At the moment three technologies for refractive ocular anomalies correction are used at the Laser Surgery Center: PRK, “LASIK” and laser thermokeratoplasty (LTK). Each of these operations is performed strictly depending on the indications individual for every particular patient.
     The essential specific feature of the Russian technology of performing photorefractive keratectomy – PRK – is that all the stages of the operation are performed by laser beam, without preliminary removal of epithelium by its instrumental mechanical scrape. It is less traumatic and allows to 1.5-2 times accelerate the period of complete epithelization of the cornea and to improve clinical functional results of the operation.
     Patient’s satisfaction by the results of PRK operation in high myopia is up to 97%, which is significantly higher than in the best foreign clinics.
     LASIK operation is the main technology being used at the Laser Surgery Center for correction of any ocular optic system anomaly.
     The main advantage of LASIK operation developed at the SI IRTC Eye Microsurgery is absolute painlessness and high precision of the impact.
     This operation is non-traumatic and guarantees quick restoration of the visual functions. The patient may return to the usual life routine already in several hours after the operation.
     Daily at the Laser Surgery Center the most experienced surgeons perform up to 30 LASIK operations.
     Today the Laser Surgery Center of the SI IRTC “Eye Microsurgery” possesses the whole range of the modern excimer laser ophthalmic units. The patient can be suggested to undergo an operation using the recent generation laser “Microscan” produced in Russia, Japanese (NIDEK EC-5000) or German equipment (MEL-70).
     Such a range of excimer laser ophthalmic units can afford only large centers same as our Laser Surgery Center.
     As an alternative to excimer laser operations at the Laser Surgery Center is successfully developed the infrared keratoplasty method (LTK) for hyperopia, hyperopic, mixed astigmatism and age-related hyperopia correction.
     For these purposes the scientists of the Center created units “LIK-100” and “OKO-1”, which exceed the world analogues in safety and effectiveness of the impact.
     All the above mentioned indicates that the modern refractive microsurgery is unthinkable without use of high-energy lasers of different spectral diapason. For the present moment the laser beam exceeds the most sophisticated surgical instrument in lack of trauma and submicron precision of the impact.
     Russian scientists were the first to found the laser refractive surgery and they continue to maintain their leading position in this extremely perspective trend in ophthalmology.
     At the Laser Surgery Center during recent 10 years were treated over 5 000 patients from different countries of the world.
     Experienced specialists work at the department of the laser treatment methods of fundus oculi and anterior eye section diseases. Laser interventions are performed aimed at coagulation, destruction and stimulation of biologic structures of the organ of vision.
          Laser coagulation is performed by Argon and infrared diode lasers in various fundus oculi pathologies:
      - diabetic retinopathy;
      - central retinal vein and its branches thrombosis;
      - angiomatose (Hippel-Lindau, Coats’ disease);
      - vasculitis (Ealse’ disease);
      - central serous choriopathy;
      - posterior multifocal epitheliopathy;
      - angioid stripes (Paget’ disease);
      - central chorioretinal dystrophy with transudative component and subretinal neovascular membrane development;
      - peripheral vitreoretinal dystrophy;
      - retinoschisis, etc.
     New methods of laser treatment are developed at the Laser Surgery Center.
At the moment for the treatment of some forms of subretinal neovascular membrane and malignant choroidal tumors is used the method of transpupillary thermotherapy (TTT) by infrared laser with the wavelength 810 nm.
     At the Laser Surgery Center is developed and introduced into clinical practice pathogenic approach in glaucoma laser treatment: open angle, closed angle secondary, congenital. In cases of decompensation after the earlier performed antiglaucomatous operation – non-penetrating deep sclerectomy a new operation of descemetogoniopuncture was developed.
Combined iridotomy was suggested for a dark colored and thick iris.
     In terminal painful glaucoma was performed transscleral cyclophotocoagulation by infrared diode laser.
     During many years at the department is successfully performed YAG-laser discission of secondary cataracts, adhesions in the anterior chamber and other operations in the anterior section of the eye.
     Aimed at therapeutic impact on the ocular tissues is used He-Ne laser in cases as follows:
      - amblyopia;
      - dry maculodystrophy;
      - partial atrophy of the optic nerve;
      - accommodation spasm;
      - corneal dystrophy (epithelial-endothelial dystrophy);
      - viral keratitis and keratopathy.
     Papillomas and other benign tumors of the eyelids are effectively removed by CO-2 laser.
     FLUORESCENT ANGIOGRAPHY DEPARTMENT performs angiographic examinations of the anterior section of the eye: conjunctiva, cornea, iris, and that of the fundus oculi: the retina, choroid in various pathology of the organ of the vision by the most up-to date equipment using films, television, digital scanning technologies with application of fluorescin and indocyanin.
     Traditions in combination with development of the new own technologies allow the Laser Surgery Center to stay at the advanced positions in the modern medical science. High qualification of the employees of the Center, most up-to-date equipment, individual approach, care and attention of medical staff help in returning patient’s perfect vision and joy of the surrounding world perception.
