On February 10, 2003 the training operating room of energetic surgery (WETLAB) has been opened at the Research Educational Center of the head-organization of the Sv. Fyodorov State Institution IRTC «Eye Microsurgery» in Moscow and a cycle in phacoemulsification of cataract has been carried out on its basis.
For the first time in Moscow the WETLAB is performed at the highest up-to-date level of technique and equipment: with the application of operating floor microscopes (OPTON production), as well as phacoemulsificators “Catarex” (OERTLI), “Legacy 20000” (ALCON), “Millenium” (BAUSCH&LOMB) and “Protege” (STORZ). All the microscopes are equipped with video-cameras and the view is transmitted through the video monitors installed in the training operating room of the energetic surgery, that allows teachers to control technique of phacoemulsification procedure of all trainees. The instructor can also notice the errors in the technology of the procedure of phacoemulsification and can help to correct it in time. Each surgeon is supplied with a set of all necessary instruments for phacoemulsification.
The 3d-level training operating room of the energetic surgery WETLAB of the EYE MICROSURGERY Complex is one of the best in the world. The full imitation of clinical phacoemulsification of cataract is achieved only with the application of the pedal control operating microscopes when the surgeons operate using both legs (one leg is placed on the foot pedal of microscope, the other - of phacoemulsificator) and both hands (in one hand there is a tip of phacoemulsificator, in another - a phakochopper). A full imitation of real surgery is achieved also with positioning the operated eyes in a mask which allows the surgeon to operate both the left and the right eye. This WETLAB is the most democratic because it is represented by the production of several firms - producers of phacoemulsificators. The doctors-trainees can try to work in each type of equipment and choose for the further practice the most optimal device. The doctors who already have in their clinics such device use mainly the same type of equipment.
8 doctors from Russian cities of Sakhalin, Voronezh, Tver, Chapayevsk, Bishkek and Vyatka, as well as two doctors from the Moscow clinic “New look” participated in the first cycle of training with lectures upon all spectrum of phacoemulsification. Each “student” mastered in all stages of the procedure and performed 30-40 original procedures of phacoemulsification in animal eyes with cataract, they also implanted the newest flexible IOLs (ACRYSOF, HYDROVIEW, FLEX) as well as the PMMA lenses.
The decision about the establishment of the WETLAB was made by Prof. Khristo P. Takhchidi, Ph.D., Director General of the Sv. Fyodorov SI IRTC EYE MICROSURGERY and all the services of the Institution have helped a lot in organizational questions. A big volume of wiring electric jobs has been executed with this purpose. The Operating Theater of the EYE MICROSURGERY Complex has render assistance in the equipping the WETLAB. All the staff of the St. Petersburg branch WETLAB, which has a long standing experience of this work, first of all with the leadersship of Director Prof. L.I. Balashevich, Dr. Takhtayev Y.V. and other personnel took the active part in organization of this job. Deputy Director General on scientific activity of the EYE MICROSURGERY Complex Dr. B.E. Malyugin took part not only in the creation but also personally the direct work of the Moscow WETLAB.
During more than 20 years Prof. V.G. Kopayeva is at the head of the Research Educational Center of the SI IRTC EYE MICROSURGERY on which basis the WETLAB of the energetic surgery of cataract is created. Presently the SPC consists of two following sectors:
1. Sector of the additional professional education which performs the advanced courses for ophthalmologists.
2. Sector of the post-graduate education - residency, post-graduate and doctorate study.
Sector of the additional professional education (headed by Dr. Legkikh L.S.) took the main part in organization and creation of the training operating hall of the energetic surgery of cataract. The lectures are prepared also by the staff of this sector (senior research worker Dr. Andreev Y.V.).
The first WETLAB level has been created by the decision of Academician S.N. Fyodorov 20 years ago in 1981 when at the advanced courses desk microscopes, almost laboratory microscopes MBS-9 ( with a short-focus distance which, nevertheless, allow to perform the radial keratotomy in animals) have been used. That period the Fyodorov’s Clinic was frequently visited not only by Russian but also by foreign ophthalmologists. More than 300 foreign doctors have been studied in these courses.
The second level WETLAB that is used by modern international courses is based on application of surgical desk microscopes without foot pedal control. Such type of the training rooml for manual surgery with operating desk OPTON microscopes works also presently at the EYE MICROSURGERY Complex. In this hall residents and participants of the courses perfect the main procedures of eye microsurgery under the microscopes.
Creation of the new high-technology training operating raam of energetic cataract surgery at the Sv. Fyodorov SI IRTC EYE MICROSURGERY means that this Clinic is developing successfully and as usually it helps to improve skill of its own ophthalmologists as well as other doctors from Russia, ex-USSR States and other countries in eye microsurgery field.
